Job summary

Ignite Rehab, operating as Metro Physical & Aquatic Therapy, is seeking a part-time in-home Physical Therapist in Essex County. The role offers benefits and a flexible schedule with about 20 visits per week.

Ideal candidates have a PT program degree and NJ licensure or eligibility. Training and mentorship are provided, and travel to patient homes is required within the assigned territory.

Qualifications

• Graduate from a Physical Therapist program.
• Licensed PT in New Jersey or eligible.
• Home health experience preferred but not required; mentor provided.
• Willingness to travel to patients’ homes within geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care.
• Educate family members, home health aides, and other caregivers on proper therapy techniques.
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Must be willing to treat geriatric patients.

Education

Physical Therapist Program Graduate

Licensed Physical Therapist in NJ or eligible
