Job summary

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from CAPTE-accredited physical therapy program.
• At least 1 year of physical therapy experience, preferably in home health; mentoring/residency options if less than 1 year.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations at least every 30 days, modify interventions as needed, and perform discharge planning.
• Other duties as assigned.
