Amedisys is seeking a compassionate Physical Therapist to provide in-home care across Sequatchie, Bledsoe and Van Buren counties in Tennessee. You will assess, treat, and reexamine patients to restore mobility and function, with discharge planning as part of the care plan.

Qualifications

• Current license to practice physical therapy in the state
• Graduate from CAPTE-accredited PT program
• One year of home health PT experience preferred
• CPR certification required

Responsibilities

• Help patients maintain, restore, and improve movement, activity and function.
• Conduct reexaminations at least every 30 days and adjust interventions as needed.
• Perform discharge planning and other duties as assigned
