Job summary

Amedisys Home Health in Tennessee is seeking a compassionate Physical Therapist to help patients regain strength and independence in the comfort of their homes. You will perform assessments, deliver targeted interventions, and participate in discharge planning across Sequatchie, Bledsoe and Van Buren counties.

Qualifications

• Current license to practice physical therapy for the assigned state.
• Graduate from CAPTE-accredited PT program.
• CPR certification current and valid.
• One year home health PT experience preferred or regional leadership approval.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Perform reexaminations every 30 days and adapt interventions as needed.
• Develop discharge plans and coordinate care with the team.
