Job summary

21st Century Home Health Services (21HHS) is seeking a Physical Therapist to deliver in-home therapy across multiple Bay Area counties. You will assess patients in their homes, create and execute plans of care, and coordinate with an interdisciplinary team to optimize recovery.

In this role, you will document promptly, educate patients and families, and participate in weekly case conferences. A California PT license and BLS/CPR are required, with on-call duties as needed.

Qualifications

• Current, unrestricted California Physical Therapist license.
• Current BLS/CPR certification.
• Valid CA driver’s license with clean driving record.
• Graduate of an accredited Physical Therapy program.
• One year of PT experience preferred; new grads considered.
• Home health experience is highly desirable.
• Ability to work as part of an interdisciplinary team.
• Strong clinical judgment and communication skills.
• Familiarity with case management principles.
• Ability to use electronic documentation and mobile health tools.

Responsibilities

• Provide hands-on therapeutic interventions in home health setting.
• Conduct home evaluations and assessments.
• Develop and implement plans of care under PT Case Manager.
• Document patient care within 24-48 hours.
• Educate patients and caregivers on mobility and safety.
• Collaborate with RN, OT, ST, HHA, MSW.
• Participate in weekly interdisciplinary case conferences.
• Maintain patient privacy and dignity.
• Meet productivity expectations.
• Assist in initial evaluations, reassessments, discharge planning.
• On-call or weekend coverage as required.
