21st Century Home Health Services (21HHS) seeks a dedicated Physical Therapy Assistant (PTA) to evaluate patients in their homes across the Bay Area and create personalized plans to improve mobility and reduce pain. You will work within an interdisciplinary team to deliver high-quality care, emphasizing empathy, patient education, and functional outcomes.

CA PTA license and CPR certification are required; in-home visits are mandatory with flexible scheduling and opportunities to develop

Qualifications

• PTA license in California and CPR certification required.

Responsibilities

• Examine each patient and develop a plan to promote movement, reduce pain, restore function, and prevent disability.
• Develop fitness- and wellness-oriented programs to prevent mobility loss.
• In-home patient care with emphasis on high-quality, compassionate service.
