Job summary

21st Century Home Health Services in the San Francisco Bay Area invites a Health Field Physical Therapist to provide in-home patient care in the Salinas region. You will conduct evaluations, design and implement treatment plans, and collaborate with an interdisciplinary team to optimize functional mobility.

The role emphasizes hands-on therapy, documentation within 24-48 hours, and ongoing professional development under supervision of a PT Case Manager. Prior home health experience is encouraged.

Qualifications

• Current, unrestricted California Physical Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver’s License with a clean driving record.

Responsibilities

• Provide hands-on therapeutic interventions including gait training, balance and neuromuscular re‑education, transfer training, therapeutic exercises, manual therapy, and pain management techniques under supervision.
• Conduct detailed patient evaluations and assessments in the home environment.
• Implement individualized plans of care under supervision of a Physical Therapist Case Manager.
• Provide timely documentation of patient care within 24-48 hours in accordance with plan of care and compliance standards.
• Educate patients and family/caregivers on functional mobility, exercises, safety techniques, and adaptive equipment use.
• Collaborate with the PT Case Manager to prepare for and assist in patient treatments and care plan updates.
• Participate in interdisciplinary coordination of services and report needs for OT, ST, HHA, MSW, and RN interventions.
