Job summary

21st Century Home Health Services Inc. is seeking an entry-level Field Physical Therapist to build core clinical skills in a home health setting. You will perform follow-up visits under supervision, with strong mentorship from leadership, as part of a growing Bay Area team serving Auburn, CA and surrounding counties.

You will join an organization recognized for patient care and staff development, working daily in patients' homes while maintaining documentation accuracy and safety standards.

Qualifications

• Current, unrestricted CA Physical Therapist license.
• Graduate of an accredited Physical Therapy program.
• 1 year of experience preferred; new grads considered.

Responsibilities

• Provide hands-on therapeutic interventions in a home health setting, including gait training, balance, and neuromuscular re-education.
• Assess patients in home environments and document care within 24–48 hours.
• Collaborate with the PT Case Manager and interdisciplinary team to coordinate care.
• Educate patients and families on mobility, safety, and use of adaptive equipment.
• Participate in on-call or weekend coverage as required.
