21st Century Home Health Services seeks a licensed Physical Therapist to deliver field-based care in patients' homes across the Bay Area. You will evaluate, treat, and educate patients with mobility impairments, working under a PT Case Manager to develop and implement plans of care.

This role requires CA PT license, BLS/CPR, valid CA driver's license, and ability to travel. Prior home health experience and teamwork with RN, OT, ST, MSW, and HHA are highly valued.

Qualifications

• Current, unrestricted California Physical Therapist license.
• BLS/CPR certification valid in CA.
• Valid CA Driver’s License with clean driving record.
• Graduate of an accredited Physical Therapy program.
• One year of PT experience preferred; new grads considered.
• Home health experience is highly desirable.
• Strong teamwork with RN/OT/ST/MSW/HHA and case management.
• Comfort with electronic documentation and mobile tools.
• Ability to work independently in a fast-paced home health setting.
• Commitment to annual compliance training and continuing education.

Responsabilités

• Provide hands-on PT interventions (gait, balance, neuro re-education, transfers, exercises, manual therapy) in home health.
• Assist patients with devices, prosthetics/orthotics, wheelchairs, and home safety equipment.
• Conduct detailed home-based patient evaluations and assessments.
• Implement plans of care under supervision of the PT Case Manager.
• Document patient care within 24-48 hours, per plan of care and standards.
• Educate patients and families on mobility, safety, and adaptive equipment use.
• Collaborate with PT Case Manager to prepare for treatments and update care plans.
• Engage in interdisciplinary coordination (OT, ST, HHA, RN, MSW) as needed.
• Use equipment and supplies efficiently while maintaining cost awareness.
• Pursue ongoing professional development and clinical trainings.
• Participate in on-call or weekend coverage as required.
• Maintain patient privacy and comply with HIPAA in documentation and care.
