Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one‑on‑one physical therapy to older adults in private residences or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence‑based treatment.
• Deliver therapy sessions that support wellness, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
