Job summary

FOX Rehabilitation is expanding in UNION, NJ, seeking licensed Physical Therapists to deliver one-on-one geriatric care in patients' homes and senior living settings.

You will conduct evaluations, create individualized plans, and provide therapy across in-home, private residence, and community settings. Flexibility to manage schedules and include telehealth enhances access and outcomes for older adults.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in-home and in senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support mobility, independence, and functional goals.
• Document effectively in FOX's EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth or virtual care when appropriate to extend access.
