Job summary

FOX Rehabilitation in Park Ridge, IL is seeking licensed Physical Therapists to deliver one-on-one geriatric care in homes or Senior Living Communities. You’ll develop personalized plans, treat patients with autonomy, and manage your own schedule while incorporating telehealth as appropriate.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one PT to older adults at home or in senior living settings.
• Perform evaluations, develop plans, deliver evidence-based treatment.
• Deliver sessions that support wellness, longevity, mobility, independence.
• Document in FOX EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care to extend access.
