Job summary

FOX Rehabilitation in Wilmette, IL is seeking licensed Physical Therapists to deliver one-on-one care to older adults in homes or senior living communities. You’ll perform evaluations, create individualized plans, and provide evidence-based treatment while maintaining your own schedule.

Telehealth options are available to extend access and continuity of care. You’ll collaborate with an interdisciplinary team and benefit from a clinician-led culture that supports growth and autonomy.

Qualifications

• Valid Physical Therapy license in IL or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• CPR certification current.
• Proficient with electronic medical records (EMR).

Responsibilities

• Provide one-on-one PT to older adults in homes or senior communities.
• Develop evaluations and plans of care and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence and quality of life.
• Document in FOX EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
