Job summary

FOX Rehabilitation is hiring licensed Physical Therapists to deliver one-on-one PT in older adults' homes or senior living communities in Gurnee, IL. You will conduct evaluations, create personalized plans, and provide evidence-based therapy to improve mobility and independence.

The role offers autonomy with flexible scheduling, opportunities for telehealth, and collaboration with an interdisciplinary team to extend care and support aging in place.

Qualifications

• Active Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence, and function.
