Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one geriatric care in Downers Grove, IL, delivering services in private homes and senior living settings. The role offers autonomy over your schedule and the option to incorporate telehealth for greater flexibility.

Responsibilities include evaluations, individualized care plans, and evidence-based therapy to improve mobility and independence.

Qualifications

• Valid Physical Therapy license in the state(s) of practice.
• Current CPR certification.
• Proficiency with electronic medical records (EMR).
• Comfort with basic computer use.

Responsibilities

• Provide one-on-one physical therapy to older adults in-home or within senior living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy to support mobility, independence, and functional outcomes.
• Document with FOX EMR in alignment with regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients and caregivers to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate to extend access.
