Job summary

FOX Rehabilitation is expanding in Wilmette, IL, seeking licensed Physical Therapists to deliver one-on-one care to older adults in private homes or senior living communities. You will evaluate, plan, and provide evidence-based therapy with a flexible schedule and emphasis on patient-centered outcomes.

Clinicians may incorporate telehealth, collaborate with a varied team, and educate patients and staff to promote safety and independence. Illinois pay ranges reflect the local market.

Qualifications

• Valid Physical Therapy license in the state of practice.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they live (private residences or senior living communities).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and functional independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
