Job summary

Fox Rehabilitation is growing in Scottsdale, AZ and we’re seeking passionate, licensed Physical Therapists to deliver exceptional one-on-one care to older adults where they live. You’ll work in private residences or Senior Living Communities, building meaningful relationships and flexible schedules.

You’ll conduct evaluations, develop plans of care, and provide evidence-based treatments. Telehealth may be integrated to extend access and ensure continuity of care.

Qualifications

• Valid Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
