Job summary

FOX Rehabilitation is growing in Scottsdale, AZ, seeking licensed Physical Therapists to deliver one-on-one care to older adults in private residences and Senior Living Communities. Build strong patient relationships with autonomy and a flexible schedule, including telehealth options when appropriate.

Responsibilities include evaluations, treatment planning, and documentation in FOX’s EMR system, collaborating with an interdisciplinary team, and educating patients and staff to promote safety and

Qualifications

• Licensed Physical Therapist in the state(s) of practice or eligibility to obtain licensure.
• Degree from an accredited Physical Therapy program (DPT).
• Current CPR certification is required.
• Proficient with EMR and basic computer skills.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to improve mobility, independence and function.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs and Fitness Specialists.
• Educate patients, caregivers and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
