Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Gilbert, SC, in private residences or senior living communities, with telehealth options available to extend access.

You will evaluate, create individualized plans, deliver evidence-based therapy, and document in FOX’s EMR. Collaborate with PTs, OTs, SLPs, and Fitness Specialists to promote function and aging in place.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence, and functional outcomes.
• Document effectively in the EMR system according to regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety, function, and aging in place.
