Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one geriatric care in homes and senior living communities across Pennsylvania. You will assess, treat, and create individualized plans of care that improve mobility and independence while building strong, lasting patient relationships.

Qualifications

• Valid Physical Therapy license in state(s) of practice or eligibility
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support wellness, longevity, mobility, and independence
• Document effectively within FOX’s EMR system per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote aging in place
• Provide telehealth/virtual care when appropriate
