FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in homes or senior living communities in Tucson, AZ. You’ll evaluate patients, develop personalized plans, and deliver evidence-based therapy with mobility and independence as primary goals.

This role offers autonomy, flexible scheduling, and the option to incorporate telehealth for expanded access and continuity of care, in a clinician-driven, supportive environment.

Qualifikationen

• Valid Physical Therapy license in the state of practice.
• Degree from an accredited Physical Therapy program.
• CPR certification and EMR proficiency.

Aufgaben

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations and develop individualized care plans.
• Deliver evidence-based therapy sessions to improve mobility and independence.
• Document therapy in FOX's EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.
• Incorporate telehealth when appropriate to extend access.
