Job summary

KeystoneCare, a nonprofit provider of in-home healthcare services, is seeking a Physical Therapist. In this role, you'll help patients strengthen mobility and maintain independence through thoughtful, personalized care.

Ideal candidates thrive in one-on-one settings and contribute to improved health outcomes. Responsibilities include assessing and providing rehabilitation services in patients' homes, collaborating with a multidisciplinary team, and supporting patients' recovery journeys.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Pennsylvania Physical Therapy License.
• Minimum two years of physical therapy experience in Home Health or similar settings.
• Experience treating geriatric patients in a home health context.
• Valid Driver's License and reliable vehicle.

Responsibilities

• Provide personalized rehabilitation services to patients in their homes.
• Assess, plan, and carry out treatment programs focused on improving mobility.
• Collaborate closely with a multidisciplinary team to support patient care.
• Guide patients through their recovery journey with compassion.
