Job summary

KeystoneCare, a nonprofit provider of in-home healthcare services, is looking for a compassionate Physical Therapist to provide personalized rehabilitation services. In this role, you will help patients regain mobility and maintain independence through one-on-one support in their homes.

The ideal candidate will have a valid Pennsylvania Physical Therapy License, at least two years of physical therapy experience, and a commitment to enhancing patient recovery and well-being through compassionate

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Pennsylvania Physical Therapy License.
• Minimum two years of physical therapy experience in Home Health or similar settings.
• Experience treating geriatric patients in a home health or similar care setting.
• Experience with post-surgical rehabilitation and orthopedic rehabilitation.
• Valid Driver’s License and reliable vehicle.
• Strong communication skills for effective collaboration.

Responsibilities

• Provide personalized rehabilitation services to patients in their homes.
• Assess, plan, and carry out treatment programs focused on improving mobility.
• Collaborate closely with a multidisciplinary team for patient care.
• Guide patients through their recovery journey with compassion.
• Help restore patients' independence through high-quality care.
