Job summary

KeystoneCare is seeking a Physical Therapist to deliver in-home rehabilitation, focusing on mobility, strength, and functional independence. You will work one-on-one with patients in their homes to support recovery and improve quality of life.

The role requires a PA PT license, at least two years of home health PT experience, and strong communication with patients and care teams. A valid driver's license and reliable vehicle are essential.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Pennsylvania Physical Therapy License.
• Minimum two years of physical therapy experience in Home Health or similar settings.
• Experience treating geriatric patients in a home health or similar care setting.
• Experience with post-surgical rehabilitation, neurological conditions, and orthopedic rehabilitation in a home health or similar care setting.
• Valid Driver’s License and reliable vehicle.

Responsibilities

• Provide personalized rehabilitation services to patients in their homes.
• Assess, plan, and carry out treatment programs focused on mobility, strength, and function.
• Collaborate with a multidisciplinary team to support patient care and recovery.
• Guide patients through their recovery journey with compassion and professionalism.
• Help restore patients' independence and well-being through high-quality therapeutic care.
