Job summary

KeystoneCare is seeking a dedicated Physical Therapist to deliver in-home rehabilitation, partnering with families and a multidisciplinary team to improve mobility and independence. The role centers on compassionate, patient-centered care in home settings across the communities we serve.

Applicants should have a PA PT license, 2+ years in home health or similar environments, and experience with geriatric and post-surgical rehab. A valid driver's license and insurance are required.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Pennsylvania Physical Therapy License.
• Minimum two years of physical therapy experience in Home Health or similar settings.
• Experience treating geriatric patients in a home health or similar care setting.
• Experience with post-surgical rehabilitation, neurological conditions, and orthopedic rehabilitation in a home health or similar care setting.
• Experience providing care in hospice-adjacent contexts.
• Valid Driver's License and reliable vehicle.
• Proof of Auto Insurance.
• Strong communication skills for effective collaboration with patients, families, and the care team.
• Excellent organizational abilities and experience managing a diverse caseload.
• Eligible to work in the United States.
• Experience with Matrix Care is a plus.

Responsibilities

• Provide personalized rehabilitation services to patients in their homes.
• Assess, plan, and carry out treatment programs focused on improving mobility, strength, and overall functional abilities.
• Collaborate closely with a multidisciplinary team to support patient care and recovery.
• Guide patients through their recovery journey with compassion and professionalism.
• Help restore patients' independence and well-being through high-quality therapeutic care.
