Job summary

All Care and Comcare Home Health is seeking a compassionate and motivated Physical Therapist to join our growing team. This role allows you to provide high-quality, individualized care in the comfort of patients' homes while enjoying a flexible schedule and supportive team environment.

The Field Physical Therapist will perform SOC evaluations, ROC, recertifications, and discharges, collaborating with an interdisciplinary team to reach patient goals and ensure optimal outcomes.

Qualifications

• Active California Physical Therapy license.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility to work across service area.

Responsibilities

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform SOC evaluations, ROC, recertifications, and discharges.
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on home exercise programs.
