Job summary

All Care and Comcare Home Health is seeking a compassionate Physical Therapist to join our growing team. This role provides high-quality, individualized care in patients' homes with a flexible schedule and supportive team environment.

As a Field Physical Therapist, you will perform SOC/ROC/Recert/DC evaluations, collaborate with the interdisciplinary team to set patient goals, and educate families on home exercise programs.

Qualifications

• Active California Physical Therapy license/registration.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes.
• Flexibility regarding work locations within service area.

Responsibilities

• Provide skilled PT services per physician orders.
• Perform SOC, ROC, Recert, and DC evaluations.
• Assess patient needs, plan interventions, and evaluate outcomes.
• Collaborate with interdisciplinary team on patient goals.
• Educate patients and caregivers on home exercise programs.
