Job summary

KeystoneCare in Pennsylvania is seeking a dedicated Physical Therapist to deliver in-home rehabilitation, helping patients regain mobility and independence through personalized, compassionate care. Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Pennsylvania Physical Therapy license.
• Minimum two years of physical therapy experience in home health or similar settings.
• Experience with geriatric patients in home health or similar care setting.
• Experience with post-surgical rehabilitation and neurological/orthopedic rehab in home health.
• Valid driver''s license and auto insurance.
• Excellent communication and organizational skills.
• Eligible to work in the United States.
• Experience with MatrixCare is a plus.

• Provide personalized rehabilitation services to patients in their homes.
• Assess, plan, and carry out treatment programs to improve mobility and function.
• Collaborate with a multidisciplinary team to support patient care.
