Job summary

KeystoneCare is looking for a Physical Therapist to provide compassionate in-home rehabilitation services. You will assess and treat patients to improve mobility and restore function while collaborating with a multidisciplinary team.

The role requires a Pennsylvania Physical Therapy License and a minimum of two years' experience in Home Health. Flexible scheduling options and various benefits are offered to support your work-life balance.

Qualifications

• Minimum two years of physical therapy experience in Home Health or similar settings.
• Experience treating geriatric patients in a home health or similar care setting.
• Experience with post-surgical rehabilitation, neurological conditions, and orthopedic rehabilitation.
• Experience providing care in hospice-adjacent contexts.
• Valid Driver’s License and reliable vehicle.
• Proof of Auto Insurance.
• Eligible to work in the United States.

Responsibilities

• Provide personalized rehabilitation services to patients in their homes.
• Assess, plan, and carry out treatment programs focused on improving mobility.
• Collaborate closely with a multidisciplinary team to support patient care.
• Guide patients through their recovery journey with compassion and professionalism.
• Help restore patients' independence and well-being through high-quality therapeutic care.
