Job summary

KeystoneCare Home Health and Hospice in Pennsylvania is seeking a licensed Physical Therapist to provide in-home rehabilitation, helping patients regain mobility and independence.

This role involves assessing and planning personalized treatment, delivering hands-on therapy, and collaborating with a multidisciplinary team to support recovery.

Requirements include an accredited PT program graduate, PA PT license, 2+ years in home health or similar care, valid driver's license and auto insurance.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Pennsylvania Physical Therapy License.
• Minimum two years of physical therapy experience in Home Health or similar settings.
• Experience treating geriatric patients in a home health or similar care setting.
• Experience with post-surgical rehabilitation, neurological conditions, and orthopedic rehabilitation in a home health or similar care setting.
• Experience providing care in hospice-adjacent contexts.

Responsibilities

• Provide personalized rehabilitation services to patients in their homes.
• Assess, plan, and carry out treatment programs focused on improving mobility, strength, and overall functional abilities.
• Collaborate closely with a multidisciplinary team to support patient care and recovery.
• Guide patients through their recovery journey with compassion and professionalism.
• Help restore patients' independence and well-being through high-quality therapeutic care.
