Ascension at Home together with Compassus is seeking a Home Health Physical Therapist to deliver in-home assessments and rehabilitation focused on restoring mobility and function. You will evaluate patients, design personalized treatment plans, and monitor progress, all within the patient’s home environment.

You’ll work one-on-one with patients, communicating with families and care teams to support safe transfers, balance, and activities of daily living, while embracing a compassionate, flexible

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Current State License as a Physical Therapist.
• Valid Driver’s License and CPR certification.
• Two years’ PT experience in acute care or rehab; home health preferred.
