Job summary

Ascension at Home together with Compassus is seeking a Home Health Physical Therapist to work one-on-one with patients in their homes, delivering compassionate rehabilitation and helping people regain function.

You will collaborate with a supportive team, adhere to standards of care, and guide patients toward independence in daily activities, such as bathing, mobility, and community participation.

Qualifications

• Bachelor of Science in Physical Therapy is required.
• Current State License as a Physical Therapist.
• Valid Driver’s License.
• Current CPR, negative TB screen and Hepatitis consent/declination.
• Two years’ experience as a Physical Therapist in an acute care or rehabilitation setting.

Responsibilities

• Provide one-on-one home health physical therapy visits.
• Help patients regain strength and independence for activities of daily living.
