Job summary

Powerback Rehabilitation To You is seeking a dedicated Physical Therapist to provide in-home outpatient rehabilitation for older adults across the Hartford area. You will assess, plan, and apply individualized PT programs in patients’ homes or living facilities to restore function and promote independence.

The role emphasizes collaboration with caregivers and rehab colleagues, safety, and versatile treatment modalities, including therapeutic exercise, manual therapy, and functional training.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• Master's degree in Physical Therapy or a related field with appropriate credentials.
• Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience.
• Degree earned outside the U.S. must be evaluated to U.S. equivalent.

Responsibilities

• Supervise PTAs, temporary licensees, students, and support personnel as required by state licensure.
• Adhere to department and patient treatment schedules; complete timely clinical and billing documentation.
• Attend patient care and staff conferences; contribute to related meetings.
• Refer to other disciplines to address comprehensive patient needs.
• Collaborate with rehab team to select appropriate therapeutic regimens.
• Adapt plan of care according to individual patient needs.
• Promote safety and remove hazards; order and maintain equipment as needed.
• Document and report outcomes; perform routine reexaminations as required.
• Educate patients and caregivers to promote independence and productive living.
• Incorporate wellness programming and community mobility assessments into treatment.

Education

Master's degree in Physical Therapy

Master's degree in Physical Therapy related field + Bachelor's degree in Physical Therapy

Bachelor's degree in Physical Therapy + 5 years of PT experience

Degree earned outside the U.S. must be evaluated for U.S. equivalence
