Qualifications

• Licensed Physical Therapist in New York or eligible.
• Home health experience preferred.
• Ability to travel to patients’ homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients in the home setting.
• Evaluate patients’ PT needs and develop individualized plans.
• Plan and implement therapy programs to restore function.
• Instruct family members and caregivers on therapy techniques.
• Provide timely documentation for each visit.
• Coordinate discharge planning with physicians and team.
