Job summary

Metro Physical & Aquatic Therapy is seeking a motivated full-time Physical Therapist to provide in-home care across Eastern Suffolk County, NY, including Medford. Base salary ranges from $95,000 to $110,000 per year with a $3,000 sign-on bonus and full benefits.

See patients in their homes seven days a week with modern, AI-assisted documentation tools and strong work‑life balance. Licensed NY PT or eligible to obtain licensure is required.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Licensed Physical Therapist in New York or eligible for licensure.
• Home health experience is a plus but not required.

Responsibilities

• Assess patients in their homes within the designated geographic area.
• Evaluate PT needs and develop individualized treatment plans.
• Create a plan of care and goals in consultation with physician and family.
• Implement customized therapy programs to restore function and independence.
• Instruct family members and aides on proper therapy techniques.
• Provide timely, accurate documentation for each visit.
• Coordinate discharge planning with patients, families and the interdisciplinary team.
