Job summary

Metro Physical & Aquatic Therapy is seeking a motivated per diem Physical Therapist to provide in-home care in Dix Hills and surrounding towns. Flexible schedule with 1099 and W-2 options, training provided for new graduates, and opportunities for professional growth in a growing organization.

Responsibilities include evaluating patients, planning and implementing customized home therapy, and documenting visits. Must be licensed in NY or eligible, with ability to travel to patient homes.

Qualifications

• Licensed NY Physical Therapist or eligible to obtain licensure.
• Graduate of an accredited Physical Therapy program.
• Home health experience preferred but not required; mentorship provided.
• Ability to travel within the geographic territory.

Responsibilities

• Evaluate patients' needs and develop personalized home therapy plans.
• Accept and treat patients in the home setting within geographic area.
• Coordinate plan of care with physician, patient, and family.
• Deliver and document therapy sessions with timely reports.
• Instruct family members and aides on therapy techniques.
• Communicate with interdisciplinary team around discharge planning.
• Ensure care complies with agency policies and professional standards.
