Job summary

Metro Physical & Aquatic Therapy is seeking a full-time Home Care Physical Therapist to deliver in-home PT services across Suffolk County, NY. The role includes evaluating patients, creating care plans, and coordinating with physicians and families.

Qualifications

Licensed Physical Therapist in NY or eligible.
Home health experience preferred.
Ability to travel to patients’ homes within the assigned geographic territory.
PT program graduate from accredited school.

Responsibilities

Accept and treat patients within your designated geographic area.
Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
Plan, prepare, and implement customized therapy programs to restore function and independence.
Instruct family members, PTAs, Home Health Aides, and caregivers on therapy techniques.
Provide timely and accurate documentation for each visit.
Communicate effectively with patients, families, and interdisciplinary team around discharge planning.
Deliver care in compliance with agency policies and professional standards.
