Job summary

Metro Physical & Aquatic Therapy seeks a licensed Physical Therapist to provide in-home care in Eastern Suffolk County under a contract arrangement. The role offers 1099 and W-2 options, flexibility, and the chance to serve patients in their homes.

All levels of experience are welcome; mentoring is available for documentation and compliance, with opportunities for professional growth within a supportive team.

Qualifications

• Graduate of a Physical Therapist program
• Licensed Physical Therapist in NY or eligible
• Home health experience preferred but not required; mentorship provided for documentation and compliance
• Ability to travel to patients' homes within the assigned geographic territory

Responsibilities

• Accept and treat patients within your designated geographic area
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family
• Plan, prepare and implement customized therapy programs to restore function and promote independence
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
• Provide timely and accurate documentation for each visit
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
• Deliver care in compliance with agency policies and professional practice standards

Education

Physical Therapist Program Graduate

Licensed Physical Therapist in New York or eligible
