Job summary

Metro Physical & Aquatic Therapy is seeking a motivated full-time Physical Therapist to provide in-home care across Suffolk County, including Lindenhurst and Dix Hills. Base salary ranges from $90,000 to $110,000 per year with full benefits and generous PTO.

Qualifications

• Active NY PT license or eligibility to practice in NY.
• Graduation from an accredited Physical Therapist program.
• Ability to travel to patients' homes within designated geographic area.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare, and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.
