Job summary

Powerback Rehabilitation seeks a Physical Therapist for PRN outpatient/home-based services in New Jersey. You will provide in-home outpatient rehab to older adults, assess needs, develop and deliver individualized therapy programs, and collaborate with caregivers to promote independence and safety.

Requirements include a valid Physical Therapy license in the U.S., a PT degree (Master's or equivalent), and driver’s license or reliable transportation.

Qualifications

• Valid Physical Therapy license in the U.S. or eligibility for licensure.
• Master's degree in Physical Therapy or equivalent; or Bachelor's degree plus 5 years of PT experience.

Responsibilities

• Provide in-home outpatient rehabilitation services for older adults.
• Develop and implement individualized PT treatment plans.
• Educate patients and caregivers to promote independence and safety.
• Collaborate with the rehabilitation team to optimize patient outcomes.
• Document clinical findings and maintain accurate records.

Education

Master's degree in Physical Therapy or Doctorate in Physical Therapy

Bachelor's degree in Physical Therapy + 5 years of PT experience
