21st Century Home Health Services (21HHS) is seeking an entry-level Field Physical Therapist to build core clinical skills in a home health setting in the Concord, CA area. You will be supervised by a Physical Therapist Case Manager and participate in weekly interdisciplinary case conferences.

The role emphasizes follow-up visits over case management, with training, mentorship, and professional development opportunities. Comfortable with electronic documentation and field-based work is essential.

Qualifications

• California PT license required.
• BLS/CPR certification required.
• CA driver’s license required.
• Graduated from an accredited Physical Therapy program.
• One year of PT experience preferred; home health a plus.
• Experience with high-acuity patients is desirable.
• Ability to work in an interdisciplinary team (RN, OT, ST, MSW, HHA).
• Familiarity with case management principles is preferred.
• Strong communication, time management, and problem-solving skills.
• Experience with electronic documentation and mobile healthcare tools.

Responsibilities

• Provide hands-on PT interventions (gait, balance, neuro re-education, transfers, exercises, manual therapy, pain management) per license and in home health.
• Assist patients with devices (prosthetics/orthotics, wheelchairs) and home safety.
• Conduct detailed home evaluations and assessments.
• Implement plans under supervision of a PT Case Manager.
• Document care within 24–48 hours, per plan and compliance.
• Educate patients and caregivers on mobility, exercises, safety, and equipment use.
• Collaborate with PT Case Manager for care updates and plan adjustments.
• Participate in interdisciplinary services and coordinate care with RN, OT, ST, MSW, HHA.
• Maintain cost awareness and proper use of equipment.
• Pursue ongoing clinical trainings and certifications.
