Job summary

21st Century Home Health Services is seeking an entry-level Health Field Physical Therapist to build core clinical skills through home visits in the Cupertino area. You will work under the supervision of a Physical Therapist Case Manager, focusing on follow-up visits and gaining exposure to company operations.

You will receive strong leadership support, mentorship, and opportunities for professional development while delivering patient-centered home health care.

Qualifications

• Current, unrestricted California Physical Therapist (PT) license.
• BLS/CPR Certification.
• Valid CA Driver’s License with a clean driving record.
• Graduate of an accredited Physical Therapy program.
• One year of experience as a PT preferred; new grads may be considered.
• Prior home health experience is desirable.
• Experience with high acuity patients is a plus.
• Ability to work in a interdisciplinary team (RN, OT, ST, MSW, HHA).
• Familiarity with case management principles is preferred.
• Strong clinical judgment, communication, time management and problem-solving skills.

Responsibilities

• Provide hands-on therapeutic interventions including gait training, balance and neuromuscular re-education, transfer training, therapeutic exercises, manual therapy, and pain management under supervision.
• Assist with devices, wheelchairs, and home safety equipment.
• Conduct home-based patient evaluations and assessments.
• Implement plans of care under PT Case Manager supervision.
• Document patient care within 24–48 hours.
• Educate patients and families on mobility and safety.
• Collaborate with PT Case Manager for treatments and updates to care plans.
• Coordinate with RN, OT, ST, MSW, HHA for required interventions.
• Use equipment efficiently while maintaining cost awareness.
• Participate in professional development and trainings.
