Job summary

21st Century Home Health Services in Vallejo, CA seeks an entry-level Health Field Physical Therapist to build core clinical skills in a home health setting. You will perform follow-up home visits with strong leadership support, focusing on hands-on therapy and documentation.

New-hire incentives for November 2025 are offered. The role requires a California PT license, BLS/CPR, and a CA driver’s license, with potential mentoring from a PT Case Manager.

Qualifications

• Current, unrestricted California Physical Therapist license.
• BLS/CPR certification is required.
• Valid California Driver’s License with clean driving record.
• Graduate of an accredited Physical Therapy program.
• One year of PT experience preferred; new grads considered.
• Home health experience is highly desirable.
• Ability to work in an interdisciplinary team (RN, OT, ST, MSW, HHA).
• Comfortable with electronic documentation and mobile healthcare tools.

Responsibilities

• Provide hands-on therapeutic interventions in home health including gait training, balance, and neuromuscular re-education.
• Assist in managing patients with assistive devices and home safety equipment.
• Conduct patient evaluations in the home environment.
• Implement care plans under supervision of a PT Case Manager.
• Document patient care within 24–48 hours.
• Educate patients and families on mobility, safety, and adaptive equipment use.
• Coordinate with PT Case Manager and care team for updates to the plan of care.
• Collaborate in interdisciplinary case conferences as required.
• Stay compliant with privacy and home health standards.
