21st Century Home Health Services is seeking an entry-level Field Physical Therapist to build core clinical skills while visiting patients in their homes in the Gilroy area. You will work under a Physical Therapist Case Manager, focusing on follow‑up visits and developing hands‑on therapy skills with strong leadership support.

The role emphasizes collaboration with an interdisciplinary team (RN, OT, ST, MSW, HHA) and requires California PT licensure, CPR certification, and a valid CA driver’s

Competenze

• Current, unrestricted California Physical Therapist (PT) License.
• Current BLS/CPR Certification.
• Valid CA Driver’s License with a clean driving record.
• Graduate of an accredited Physical Therapy program.
• One year of experience as a Physical Therapist preferred.
• Home health experience desirable.

Mansioni

• Provide hands‑on therapeutic interventions in home health settings.
• Conduct detailed patient evaluations and assessments in the home environment.
• Implement individualized plans of care under supervision of a PT Case Manager.
• Document patient care accurately within 24–48 hours.
• Educate patients and families on mobility and safety techniques.
