Job summary

21st Century Home Health Services (21HHS) seeks an entry-level Field Physical Therapist to build core clinical skills through home visits in Elk Grove. You will work under the supervision of a Physical Therapist Case Manager and participate in interdisciplinary care plans with a strong focus on hands-on intervention and patient education.

Qualifications

• Current, unrestricted California Physical Therapist license.
• Current BLS/CPR certification.
• Valid CA Driver’s License with a clean driving record.
• Graduate of an accredited Physical Therapy program.
• One year of PT experience preferred; new grads may be considered.
• Home health experience is highly desirable.
• Experience with high acuity patients is a plus.
• Ability to work in an interdisciplinary team (RN, OT, ST, MSW, HHA).
• Comfort with electronic documentation and mobile healthcare tools.

Responsibilities

• Provide hands-on therapeutic interventions including gait training, balance and neuromuscular re-education, transfer training, therapeutic exercises, manual therapy, and pain management within a home health setting.
• Assist in managing patients with assistive devices and home safety equipment.
• Perform detailed patient evaluations and assessments in the home environment.
• Implement individualized plans of care under supervision of the PT Case Manager.
• Document patient care accurately within 24–48 hours per plan of care.
• Educate patients and families on mobility, exercise, safety, and adaptive equipment usage.
• Collaborate with the PT Case Manager to prepare for treatments and update care plans.
• Participate in interdisciplinary coordination and report needs for OT/ST/HHA/RN as appropriate.
• Maintain efficiency with equipment and supplies, while controlling costs.
• Engage in ongoing professional development and skill refinement.
• Attend initial evaluations, reassessments, and discharge planning with the Case Manager PT.
• Participate in on-call or weekend coverage as required.
