Job summary

Millennium Physician Group in Florida (Sarasota County) seeks a Physical Therapist to join our Home Health Team, delivering in-home evaluations and treatment plans to improve mobility and independence. You will collaborate with physicians and an interdisciplinary team, educate patients and families, maintain clinical documentation, and ensure regulatory compliance.

Ideal candidates hold FL PT licensure, at least 1 year of clinical experience in home health or related settings, and demonstrate

Qualifications

• Licensure: Current and valid Physical Therapist license in Florida.
• Experience: Minimum of 1 year of clinical experience as a Physical Therapist, preferably in home health or related setting.
• Skills: Strong clinical skills, excellent communication and interpersonal abilities, patient-centered approach to care.
• Professionalism: Strong work ethic, reliability, commitment to high-quality care.
• Transportation: Reliable transportation to visit patients in their homes.

Responsibilities

• Patient Assessment: Conduct comprehensive evaluations of patients' physical abilities, limitations, and functional needs to diagnose impairments and develop treatment plans.
• Therapeutic Interventions: Implement manual therapy, therapeutic exercises, and gait training to improve mobility, strength, balance, and pain management.
• Patient Education: Educate patients and families on body mechanics, home exercise programs, and injury prevention strategies.
• Collaboration: Work with physicians, nurses, OTs, and other healthcare professionals to develop care plans and outcomes.
• Documentation: Maintain accurate and timely documentation of evaluations, plans, progress notes, and discharge summaries.
• Compliance: Adhere to HIPAA and Medicare regulations.
