Millennium Physician Group is seeking a Physical Therapist to join our Home Health Team in Lee County, Florida. You will evaluate patients, develop individualized treatment plans, and provide interventions to improve mobility and independence while collaborating with an interdisciplinary team to ensure high‑quality care.

Responsibilities include documentation, patient education, and adherence to HIPAA/Medicare regulations.

Qualifications

• Florida license in good standing required.
• Minimum of 1 year clinical PT experience, preferably in home health.
• Strong clinical skills and patient‑centered approach.
• Excellent communication and interpersonal abilities.

Responsibilities

• Conduct patient evaluations and develop individualized treatment plans.
• Provide manual therapy, therapeutic exercises, and gait training.
• Educate patients and families on home exercises and injury prevention.
• Collaborate with physicians, nurses, and therapists to coordinate care.
• Maintain accurate clinical documentation and ensure regulatory compliance.
• Adhere to HIPAA and Medicare regulations.
