Job summary

Millennium Home Care in Florida seeks a dedicated Physical Therapist to provide in-home skilled PT services under physician orders. You will perform assessments, develop treatment plans, and supervise support staff to help clients regain function and manage pain in their homes.

You will educate families, coordinate care with physicians, and document progress, ensuring adherence to care protocols and discharge planning. Requires Florida PT license and previous home health experience.

Qualifications

• Must hold a current Florida PT license.
• Experience in in-home health is preferred.
• Ability to educate and supervise family and client.

Responsibilities

• Provides initial and ongoing assessments including OASIS.
• Establishes and revises treatment plans with physician input.
• Supervises PT Assistants and paraprofessionals monthly.
• Educates and counsels family and client on therapy.
• Instructs on devices and equipment use.
• Prepares discharge summaries and progress notes.
