Qualifications

• Active Florida Physical Therapist license.
• Minimum 6 months of hospital, SNF, or rehab experience.
• Strong clinical, communication, and interpersonal abilities.
• Reliable transportation to visit patients at home.

Responsibilities

• Assess patients' physical abilities and develop individualized treatment plans.
• Apply manual therapy, therapeutic exercises, and gait training.
• Provide functional training and ADL independence strategies.
• Educate patients and families on home programs and injury prevention.
• Document evaluations, progress notes, and discharge summaries.
