Job summary

Millennium Home Care is seeking a full-time Physical Therapist to provide in-home evaluation and treatment. You will design individualized plans and work with a multidisciplinary team to improve mobility and function for patients in their homes.

Applicants must hold Florida PT licensure with at least 6 months of relevant experience, strong communication skills, and reliable transportation to patient homes. Competitive benefits and flexible scheduling are offered.

Qualifications

• Florida license as a Physical Therapist is required.
• Minimum 6 months of experience in hospital, skilled nursing facility, or rehab.
• Strong clinical and interpersonal skills; patient-centered care.

Responsibilities

• Patient Assessment: Evaluate patients' physical abilities and needs to plan care.
• Therapeutic Interventions: Apply manual therapy, exercises, and gait training.
• Functional Training: Teach compensatory strategies and device training for independence.
• Patient Education: Instruct on body mechanics, home exercises, and prevention.
• Collaboration: Work with physicians, nurses, and therapists on care plans.
• Documentation: Record evaluations, plans, progress, and discharge notes.
• Compliance: Follow HIPAA, Medicare, and regulatory guidelines.
